Terms and Conditions:

By accessing and using this service, you accept and agree to be bound by the terms and provision of this agreement. In addition, when using these particular services, you shall be subject to any posted guidelines or rules applicable to such services. Any participation in this service will constitute acceptance of this agreement. If you do not agree to abide by the above, please do not use this service.

Users of this service must agree to collection and usage of personal information for the purposes described in our privacy policy.

Payment is required at the time of service unless alternatives are arranged prior to the service provision. Payment can be made by cash, eftpos, HI-CAPS, credit card or cheque.

Southern Downs Dental Pty. Ltd. reserves the right to change these conditions from time to time as it sees fit and your continued use of the service will signify your acceptance of any adjustment to these terms. You are therefore advised to re-read this statement on a regular basis.

Southern Downs Dental reserves the right to refuse service to those who do not accept or who breach the terms and conditions detailed herein.

This offer is only available to clients who use the service between the 1st July 2017 and 31st August 2017. Therefore limited spaces will be available for appointments.

Applicable item numbers include: 011/012 – Examination, 111/114/115 – removal of plaque/calculus (clean), 121 – application of remineralising agent (Fluoride or CPP-ACP application), 022 – intra-oral PA or Bitewing (X-ray).

Limit one claim per client for 011/012/111/114/115/121 for the duration of the promotional period. Limit 4 claims per 022 per client for the promotional period.

In order to be eligible for this promotion, if you have private dental cover, it must have been at least 6 months since your previous claim through your health fund on 011/012/111/114/115/121 to claim on these items again. This includes any claims made through any other dental practice.

Of the services included in the promotion, those that are offered will be at the discretion of the treating dental professional who will take in to account the individual health requirements of each client and the risks and benefits, to the client’s health, of accepting the service. In addition, services to which the client does not offer valid verbal consent, will not be provided.

If a client has existing valid private dental cover with any private health fund, the following services will be provided at no out-of-pocket cost (i.e.: no GAP): maximum of one per client during the promotional period of these item numbers – 011/012 (exam), 111/114/115 (clean), 121(fluoride or CPP-ACP treatment) and maximum of 4 each during the promotional period of the item 022 (X-ray).

If a client has no private dental cover they will be entitled to a 50% reduction in the standard fees which are listed as follows for these item numbers:
